26/27 PreK Qualification Form
Before you begin this online form, please make sure you have the CORRECT documents SCANNED into PDF format. Pictures of documents and screenshots may not be accepted.

Effective June 1, 2026, this form will accept PDF files only. Other formats, including .jpeg, .jpg, .png, and .docx, will not be accepted.

Documents you should have ready to upload in PDF format:
1. ASPIRE Application (embedded below)
2. Child's Birth Certificate, OR Passport, OR Certificate of Live Birth
3. One of the following for Income Verification
  - Previous Year Tax return
  - One month of paystubs from the latest previous month
  - W2
  - Notarized letter stating no income
We NO LONGER ACCEPT the following for Income Verification:
- Scholarship documentation
- SNAP Cards or benefit letter
- WIC Cards
- Medicaid Cards or benefit letter
